Develop interview questions to gather information about the role of the provider in the community and the health issues faced by the chosen community.
Complete the “Provider Interview Acknowledgement Form” prior to conducting the interview. Submit this document separately in its respective drop box.
Compile key findings from the interview, including the interview questions used, and submit these with the presentation.

Create a PowerPoint presentation of 15-20 slides (slide count does not include title and references slide) describing the chosen community interest.
Include the following in your presentation:
Description of community and community boundaries: the people and the geographic, geopolitical, financial, educational level; ethnic and phenomenological features of the community, as well as types of social interactions; common goals and interests; and barriers, and challenges, including any identified social determinates of health.
Summary of community assessment: (a) funding sources and (b) partnerships.
Summary of interview with community health/public health provider.
Identification of an issue that is lacking or an opportunity for health promotion.
A conclusion summarizing your key findings and a discussion of your impressions of the general health of the community.

Community Health Provider’s Role and Experiences Within The Community
Introduction
This presentation discusses findings on the interview with a community health provider regarding his role and experiences within the Downtown Phoenix community in Arizona. The areas covered in this presentation are description of community and community boundaries including the people and the geographic, geopolitical, financial, and educational level. The community’s description also covers ethnic and phenomenological features of the community, as well as types of social interactions. In addition, the community description encompasses common goals and interests, and barriers, and challenges, including any identified social determinates of health. Besides, the presentation presents a summary of community assessment by focusing on funding sources and partnerships. Moreover, this presentation entails a summary of interview with community health provider and identified an issue within the Downtown Phoenix community that is lacking or an opportunity for health promotion within this community. The presentation concludes by summarizing the key findings and discussing my impressions of the general health of the Downtown Phoenix community.
Description of Downtown Phoenix Community and Community Boundaries
The People and the Geographic
Based on the findings from the interview, the Phoenix region has a population of about 1.7 million individuals. This community comprises individuals from various races including Whites, African Americans, Latinos, and Asians among other races. The number of males and females within this community is almost equal. Majority of the population are aged18 years and over, with individuals aged 65 years and over accounting for about 10% of the entire population.

The Geographic
In relation to geographic aspect, Phoenix is the capital city of Arizona, which is situated within the south-central portion of Arizona. This region has a land area of about 517 square miles. Majority of the population comprises Christians.

Geopolitical
Based on the interview outcomes, it can be noted that Phoenix residents are welcoming to individuals from other states and countries. This region is predominantly republican, with the Phoenix city being inclined towards democrats. The region is considered one of the more conservative urban counties within the U.S.
Description of Downtown Phoenix Community and Community Boundaries
Educational Level
From the interview outcomes, it can be noted the educational level of Phoenix residents ranges from high school level to the doctorate level. There is a small percentage of individuals with high school educational level, which the community health worker estimated to be around 18%. The rest of the population boasts of high school educational attainment and over. Many individuals within the community have bachelor’s degree, with few individuals having educational qualifications above bachelor’s degree.

Ethnic and Phenomenological Features of the Community
In relation to ethnic features, the community health worker stated that English is the predominant language of the Phoenix region and that Christianity is the dominant religion. When it comes to phenomenological features of the community, the interviewee stated that violent crime is the major source of stress, with crime rates being 34% higher than national average. The community health worker added that many individuals seek assistance with stress management from support groups with few individuals focusing on hotlines for assistance with stress management.
Conclusion
In conclusion, the interview with the community health worker was a success. The major findings from the interview were the Phoenix region being a multicultural society with majority of the population being Whites, and Christians, and with educational levels above high school. The findings also revealed that there exists a range of funding sources and partnerships within the community, as well as low unemployment levels. The phoenix community can be considered conservatives. Violent crime is among the major stressors, with many community members seeking assistance from community support groups.
