Identify the topic and the nursing practice issue related to this topic. (This MUST involve a nursing practice issue.)

The topic is the development of a safety principle to reduce nursing errors in medical facilities. Inaccuracies are all too prevalent in hospitals.  Inaccuracies have negative implications that put the wellbeing of most patients in danger of disease and even death.


Briefly describe your rationale for your topic selection. Include the scope of the issue/problem.

In hospitals, mistakes are expected. Mistakes have resulted in the deaths of millions of patients worldwide. Inaccuracies are triggered by a mixture of factors, including inattention, exhaustion, and recklessness. Nurses can devise measures that minimize or eradicate errors in nursing training.












Star Point 2:  Summary (Evidence to support the need for a change)

Describe the practice problem in your own words and formulate your PICOT question.

The perform setback is characterized by a growing incidence of errors in hospital environments. Missed treatment, patients’ hands-off, diseases, crashes, and drug failures are all examples of errors. Although mistakes can be catastrophic, nothing has been done to foster a safety belief in clinical environments and enhance health satisfaction.

Protection of sick people is one of the greatest important aspects in promoting excellent healthcare. The problem is one of the variables that most health organizations around the world keep a close eye on. Because of the essence of their duties and their hands-on interaction with patients, nurses are important in promoting a safe culture. A few of the patient welfare predictors mentioned include error communication, continuous response, hospital administrative support, supervision priorities, and cooperation in resolving potential faults.


Outline one or two evidence-based solutions you will consider for the trial project.

Hospital administration must create processes and procedures that improve the principles of patient protection. They should also strive to build management capability to facilitate continuous communication, constructive organization performance, collaboration, and a fault-free atmosphere.






Star Point 3:  Translation (Action Plan)

Identify care standards, practice guidelines, or protocols that may be in place to support your intervention planning (These may come from your organization or the other sources listed in your Summary section in Star Point 2).

The availability of rigorous student-centered and interactive preparation and monitoring components that improve nurses’ abilities, attitudes, and knowledge while also raising nurses’ understanding of the different methods to prevent faults promotes patient wellbeing.

List your stakeholders (by title and not names; include yourself) and describe their roles and responsibilities in the change process (no more than 5).

Patients should cooperate fully with the clinicians and be eager to point out any mistakes that the clinicians can make.

Management is interested in implementing policies and strategies that foster a sense of security in the healthcare atmosphere.  They also train and supervise employees to ensure that the formulated methods are followed.

Nurses have direct interaction with patients. Their job is to put the established techniques into action to reduce the percentage of errors.

What specifically is your nursing role in the change process? Other nursing roles?

Nurses should disclose any mistakes or close omissions to the appropriate experts so that they can assess the situation and devise sensible precautions in the future. They should also disclose any potentially hazardous materials in their profession, such as defective equipment, through collaborative practice. They should use action checklists in any situation to make sure that nothing is overlooked.

Since drug errors are so prevalent in medical care centers, pharmacists play a vital role in promoting a safe culture.

Nurse leaders are in charge of supervising nurses and advocating for reforms to be made.


What type of cost analysis will be needed before a trial?  Who needs to be involved with this?

The primary expense is in the preparation of the affected parties. During the trial phase, eight pharmacists and nurses will be educated. The majority of the expense is borne by the advertising campaign, including printing and disseminating worksheets, posters, flashcards, and creating video tutorials.






Star Point 4: (Implementation)

Describe the process for gaining permission to plan and begin a trial.  Is there a specific group, committee, or nurse leader involved?

The nurse representatives look for official approval from the hospital administration. The nurse leaders deliver a letter to the administration outlining its consequences and adopting the appropriate intervention steps. The intervention steps are also mentioned, as are the perceived outcomes from the operation.

Describe the plan for educating the staff about the change process trial and how they will be impacted or asked to participate.

Transformation may be counterproductive if good contact is not maintained. The parties concerned must be educated on making improvements to increase patient security through error mitigation.  The training program should be compelling enough to ensure that all stakeholders recognize the need for improvement and, as a result, accept the process of change.

Outline the implementation timeline for the change process (start time/end time, what steps are to occur along the timeline).

The data would include a plan for determining if the implemented improvements have decreased treatment faults in the hospital. The information would also shed light on likely means to boost the state even further.
