Practicum Journal

Within the past four weeks, in the course of working with various patients, as well as, medical professionals, I have encountered numerous learning opportunities. The experiences have accorded me the chance to assess myself in relation to my professional goals. An experience that particularly stood out for me was with regard to working with an elderly diabetic patient.

The patient was brought to the hospital for his regular check-up and I realized that he had a dry mouth and dry eyes which was an indicator of dehydration. When I asked him his name and where he lives, I realized that he was experiencing incontinence and confusion. Because of these symptoms, coupled with the fact that he was overweight and had a leg injury from an accident, I saw it fit to conduct a diabetes diagnosis. Based on the test results, it was clear that the patient had type 2 diabetes. I gave the patient a dose of metformin and when he was calm, I informed him of the diagnosis. I then provided him with adequate information regarding the dietary and lifestyle changes that he will need to make. In line with my professional goals, I decided to give keen follow-ups through phone calls. I am glad I was able to assess the signs and symptoms and give an accurate diagnosis.

Practicum experiences provide nurses with the opportunity to assess themselves in relation to their individualized professional goals. I am glad I got the chance to accurately assess the signs and symptoms and give the patient a correct diagnosis.
