Multiple Chronic Conditions result into a significant management burden for the American Healthcare Systems. The U.S. Department of Health and Human Services (HHS) has, therefore developed a health control programs to enhance management of MCC as direct efforts to reducing this health burden (Parekh et al., 2014). The vision of health department in providing care for managing chronic patients is Optimum Health and Quality of Life for Individuals with Multiple Chronic Conditions (Ward et al., 2014). The focus on this vision led to development of a framework that further elucidated four different interdependent domains framework for the good of patients.

  • The framework strengthens the healthcare alongside other public health systems to foster key system changes for improving general health among all individuals undergoing multiple chronic conditions.
  • Empowering patients to utilize proven self-care management services and programs through development of health promotion models for addressing associated risk factors and chronic conditions challenges.
  • Equipping healthcare providers with adequate and quality tools, relevant interventions and health information to promote public health, health care and other related social services workers who work to better quality of care delivered to individuals facing this problem.
  • Finally is supporting all targeted researches concerning MCC individuals by applying relevant and effective interventions for filling existing knowledge gaps and increasing knowledge capacity of care providers on how well to manage this condition while applying applies relevant systems for proper management of patients (Ward et al., 2014).

Objectives for directing Chronic Condition Management in U.S

Each strategic framework has been accompanied by specific set objectives for directing chronic conditions management in U.S. respectively.

  • To identify evidence-based models for individuals with MCC for improving coordination in care and address existing gaps in care coordination. This has been accelerated by several health models like provider communication cooperation and patient centered multidisciplinary care to ensure direct incorporation of public health in community resources ((Parekh et al., 2014)).
  • To facilitate self-care management through generation of evidence based practices for replicating and translating such programs into complete multiple settings in homes, work, healthcare units or assisted living for improving health status of the MCC.
  • To enhance health professionals training programs among social services, health and public care professionals to ensure they are readily influenced and prepared for blending into practices, since most health care professionals feel more comfortable under key chronic care competencies.
  • To increase existing external validity of trials to ascertain treatment interventions on drugs, lifestyle modifications, devices and alternative medicine to ensure patients and providers understand existing comorbidities interactions while limiting exclusion as a result of large population in clinic trials for preventing adverse health events and poor outcomes (Ward et al., 2014).
