    Accident Report

    According to the Health and Welfare at Work Act of 2005, employers are required to identify workplace hazards under their control for safety purposes (Love et al., 2019). Identifying hazards helps ensure that the risks and harm that may occur in case of an accident are well tackled. In a construction site, it is the role of the Safety Director in charge of the construction management firm to conduct a thorough investigation regarding the cause of any accident that occurs on-site and file a report to facilitate a subsequent action plan (Love et al., 2019). This essay analyses a case study of an accident reported at a high-rise condominium project where two workers were injured.

    Investigation of the Accident

    As the Safety Director in charge of the Construction Management Firm, it is my responsibility to conduct an investigation to determine the events that led to the incident at the site (Love et al., 2019). An accident can be defined as an unpleasant event or situation that results in damage of property, injury, or even death (Love et al., 2019). For a better and in-depth understanding of the safety hazards that are linked to the high-rise construction projects, various steps will be followed in the process of accident investigation to make sure that the likelihood and future reoccurrence is reduced.

    Step 1: Assessment of the Injury

    Conducting an injury assessment will be the first step to undertake. Examining the severity of harm, or injury should come first so that the necessary medical attention is offered on time (Lee et al., 2016). In the case of the high-rise condominium project accident, I will start by examining the workers with the head injuries and onsite cuts and then file a brief report to help determine the kind of medical assistance that will be offered to the injured individuals.

    Step 2: Seeking Treatment and Medical Attention

    The next step will include calling for emergency medical assistance to ensure that the injured employees, especially the ones with severe head injuries, are transferred to a healthcare facility for further treatment (Canham et al., 2018). Meanwhile, I will offer any form of first aid that can help stabilize the injured workers. In addition, I would also give the workers some forms that they should fill to facilitate a smooth return-to-work process and as well ensure that they recover well. Some forms that need to be filled include a copy of the job description of the employee, a copy of the policies of the organization regarding workplace accidents, a medical consent form, and a return-to-work form, which should be signed by a qualified medical practitioner (Lee et al., 2016).

    Step 3: Conducting an Interview with the Witnesses and Injured Workers

    To help understand the cause of the accident as well as the particular incidence, I will interview the witnesses who were on site when the accident occurred. I will also interview the two workers who were injured to get their side of the story. The interviews will enable me to gather information about the risks present at the site and prepare a report, which will facilitate an action plan for preventing the future occurrence of such incidences (Love et al., 2019).

    Step 4: Fact Findings and Analysis

    Fact-finding and analysis entail scene investigation (Canham et al., 2018). This involves taking photographs of the site particularly the specific place where the accident occurred. Also, photographs of the equipment and other objects that were involved in the act are taken (Canham et al., 2018). Safety measures are also taken to ensure the safety of other employees and personnel involved in the fact findings and analysis   

    Step 5: Compensation

                Compensation of the affected employee under the correct and relevant channels should be followed under this stage. Actions to be taken include contacting and reporting the accident to the insurance company (Love et al., 2019). Proper statements documents and files supporting the process of compensation such as the medical statement should be submitted.    

    Step 6: Follow-up

    This stage involves making a follow-up on injured and affected workers. The victim should be visited in the hospital and their recovery process is also ascertained (Love et al., 2019). Also, offering encouragement and advice is highly encouraged in this stage.

    Step 7: Corrective Action

                This is the last step, which involves the execution of affirmative action, aiming at avoiding and preventing the future occurrence of such a scenario. Also, enhancement and improvement of the whole operation and the firm at large are part of the goals kept in mind when implementing this step (Canham et al., 2018). To address any violation of the work conduct by any employee, a violation policy is followed to the later to arrive at the revenant corrective action (Canham et al., 2018). To reduce the rate of accidents and improve safety are a priority when selecting a course of action to be taken.

     

     

    Accident Incident Reporting and Investigating: Accident Incident Report Form

                An incident investigation and reporting form entails the steps for notifying, supervision, reporting, and investigating every accident that has occurred on the site (Canham et al., 2018). The report is then filled with the investigation process in mind. To complete the form, six steps have to be followed. First, the log incident is initiated. Then documentation of the type of accident follows. In this case, any injury obtained in the line of duty is recorded.  Also, the time and date should be recorded under this step. Thirdly, a vivid description of the occurrence of the accidents is done, including a photograph. Under the fourth step, a witness is identified and linked with the victim (Canham et al., 2018). Thereafter, an analysis and examination of the root cause follow. Lastly, the accident is logged with the information of the step before (Canham et al., 2018).  A sample of the form that can be filled in is as indicated;
