- Evaluate Outcomes – showing the expected outcomes and how you
would evaluate them (objectively and subjectively). These should
relate to your Goal.
- Reflect on Process and New Learning – showing how you would
reflect on the process of dealing with this particular nursing issue
and what you have learnt from it.

Assessment Task 2: Care Planning
Introduction
Establishment of an effective care plan is an integral process in the nursing discipline, as it ensures the improvement of patients’ lives. This paper discusses the nursing provision that follows on from previous identification of the nursing priorities for Thi Nguyen, an 81-year old widower diagnosed with Parkinson Disease four years ago. The paper accomplishes this goal by focusing on the nursing priorities and the concept map identified in assessment task 1. Four nursing priorities or considerations for Thi are addressed in this paper. They include management of Thi’s tremors, management of Thi’s medication, management of Thi’s social interaction, and management of Thi’s hyperthyroidism and Parkinson’s disease. Each of the four considerations will be addressed in relation to four subheadings that include goals establishment, action taking, evaluation of outcomes, and reflection and learning. These considerations are explored in light of Miller’s Functional Consequences Theory of Healthy Ageing as a theoretical framework and Levett-Jones' Clinical Reasoning Model, which are employed in guide nursing care planning, interventions and evaluation of care for Thi. These undertakings will ensure the development of an effective care planning that can contribute significantly to the improvement of Thi’s quality of life based on the areas of his life that needs management.
Management of Thi’s Tremors
Clinical reasoning and Miller’s framework will be employed in all planning for all the four priorities. According to (), clinical reason is a process that always leads to a spiral or series of linked clinical encounters. It involves the collection of cues, processing of information, comprehension of the patient’s situation or problem, planning and implementing interventions, evaluating outcomes, and reflecting on and learning from the process. Miller’s framework takes into consideration two functional consequences, which are the negative consequences and positive consequence. Negative consequences detract from a person’s level of independence and function. These consequences often occur from combination of disease impact and age-related changes. On the other hand, positive consequences contribute to a person’s level of independence and function, and always occur from internal actions or automatic actions.
