    Maeve, a 62-year-old widow, presents with an elevated papule that has a pearly appearance with a little crusting on her back above her bra strap. It’s about 8 mm and brown. She states that she didn’t even know it was there until she was wearing a bathing suit recently, and someone at the pool suggested that she have it checked out by a doctor. Since she’s now aware of it, she states that it itches.

    Physical extermination is a routine test offered by the primary care provider (PCP) in determining the overall condition of the body. This extermination aims at addressing the ongoing pains or signs as experienced by the patient. This paper will seek to discuss the type of questions, type of physical extermination elements, and designing a treatment plan for Maeve, a 62 years old widow who presents herself with a pearly papule. The paper will also discuss additional testing to be performed differential and working diagnosis in addition to the inclusion of a complementary and over the counter (OTC) therapy.  

    Upon Maeve's arrival at the facility, I would ask her about additional symptoms and pain she is experiencing, recent tests undertaken, and OTC drugs or herbal supplements used. Further, I will ask the patient names of PCP she had seen, medical treatment history, and the presence of an implant in her body. Lastly, I will ask the patient the last time she used a painkiller or any other medication (Dunphy & Winland, 2001). After responding to the patient's questions, I will conduct skin extermination to determine growth or other changes that may be present on the skin. To gather adequate information for a treatment plan, I will include elements of the shape of the papule, size, and growth characteristics that may be contagious. During this exercise, I will be keen to determine if the growth is cancerous. 

    In the event of any suspicion from the physical extermination, I will consider using a dermatoscope to obtain finer details from the papule's magnified features. This will then be accompanied by a biopsy where a smaller sample of the section is taken for laboratory analysis to determine the extent of a papule on the skin; however, from the initial information provided by dermatoscope, the removal of the tissue sample may not be necessary thus a photograph of the affected section of the skin will be more appropriate for further analysis. In such a case, I will explain to the patient to allow time for analysis, which is about a week. The problem will be identified from the outcome of the analysis, followed by a well-designed treatment plan. 

    The treatment plan is designed depending on biopsy or the findings revealed by skin photographs upon the analysis. Therefore, the treatment plan can be of smaller or more invasive procedures (Ursel, 2020). For basal cell carcinoma, the minute growth can be eliminated by curettage or electrodesiccation. In the event it is more invasive, the treatment plan will include surgery to remove the melanoma. Additional administration of over the counter drugs is necessary for pain management, for instance, reducing itching on the patient's body. However, it is essential to consider appropriate prescriptions and the after-effects of specific painkillers to the patients and other underlying health conditions that may affect OTC's effectiveness.

    Patient treatment is a comprehensive activity that involves different players. From the doctors' list and management provided, I will encourage the patient to keep in touch with her PCP. Further, I will encourage Maeve to share the test findings and the diagnosis to their closest PCP for close monitoring of medical response to the recommended treatment type. On the other hand, I will invite other doctors to aid in managing her condition.

Assessing Risk and Reward

The Nissan Motor Company is one of the leading automobile makers in the world. The Japanese carmaker has primarily enjoyed a successful run, allowing it to enter various regional and international markets such as the United States. However, the changing business environment was not favorable to the company in 2019. Notably, the cooperation recorded losses amounting to 7.8%. The experience pushed the management into making tough decisions, requiring almost all of its North American workforce to go for unpaid leaves.

In late 2019, the company announced that the decline in sales necessitated a two-day unpaid leave for the North American workers. The stated days for the vacation were January 2 and 3rd    (Chicago Tribune, 2019).  Notably, this move was a crucial decision for the company because of its conflicting impacts. Whereas on the positive side, it could help the firm minimize expenses, it threatened to affect the public perception of the company regarding employee welfare.

The rewards for the decision involved cutting expenses by not paying the workers on leave, which eventually would translate into reduced expenses. Another reward was that the decision could allow the company to optimize performance by evaluating employee performances then developing new milestones. However, on the low side, the company risked affecting its public image and brand name, especially in the North American market. As per Chakravorti (2010), the way an organization treats its employees influences the firm's public perception. Thus, Nissan risked eliciting a negative public perception. With a distorted public image, the company could fail to revive its declining sales.

I would have advised the management of Nissan to utilize the Predictive Analytic perspective in determining the right decision to take. Ideally, the approach tries to predict what might happen in the future if particular decisions or actions are undertaken at the moment (Traymbak & Aggarwal, 2019). Looking at the situation at Nissan, the company needed to develop a goal such as increasing sales. After that, they would have made decisions aimed at realizing the set goal. In this regard, the predicted outcome could give the management an overview of whether more risks existed or significant rewards could be realized.
