Case Study 2
The purpose of this paper is to discuss Mr. P, a 76-year-old male, case study who was hospitalized for congestive heart failure. This paper provides an identified approach to care specific for Mr. P’s condition, treatment plan, patient and family education, and a teaching plan. A complete evaluation of the patient and reconsideration of current medications is indicated for this patient.
The approach to care appropriate for Mr. P is a patient-centered approach to care. The patient-centered approach acknowledges patient’s preferences, emphases on partnerships and collaboration, promotes flexibility, and improves quality and safety of healthcare (Delaney, 2018). In this approach Specific issues that contribute to recurrence of congestive heart failure symptoms such as poor adherence to manage his medications and poor adherence to dietary restrictions should be addressed through this approach. Patient-centered care focuses of the goals and desires, personal needs, values and culture, and inclusion of the patient’s preferences in care (Kredo et al., 2016).
The recommended treatment plan is as follows. First of all, a multidisciplinary team should perform a re-assessment of the patient for known conditions and comorbidities should be done so as to determine on whether the current medications in use are still indicated or not. In addition, the current medications under use by the patient ought to be evaluated for their safety, efficacy, adverse events, and/or drug-drug compatibility. Polypharmacy issues should be addressed. The patient should be administered only essential mediations. A nurse has an important role of reconciling prescribed medications (Thomas, Seifert, & Joyner, 2016). The second plan is provision of a comprehensive patient education program to tackle poor adherence issues. In addition, physical exercises are indicated for good prognoses of the existing conditions. The third solution is combination of a dietary plan and physical exercises.
The patient and his wife require tailored education and counseling to help them manage their diet and improve medication adherence. Counseling is indicated for the patient to help cope with pathological change; whereas, his wife requires counseling so as to manage dependency on her to complete his activities of daily living and gather for increased financial demands due to disease and other financial responsibilities that was gathered for earlier by the patient. Education approaches appropriate for this patient and the primary caregiver, who happens to be his wife, include use of audio-visual methods, verbal instructions, and demonstrations (Marcus, 2014).
The teaching plan is meant for both Mr. P and the wife. Both the patient and his wife need to be involved in a patient-focused education session. The education sessions will be undertaken in two weeks. I week 1 (three session; 1 hour each), the patient and his wife will be educated about the usefulness of each prescribed agents and the importance of medication adherence. Likewise, in week two the patient will be taken through the impacts and/or relationship between diet and physical activity with prognosis of congestive heart failure. The intent ion is to make Mr. P’s wife supportive as well as to improve the patient’s adherence to medications, dietary changes, and physical exercises.
