Case 1

Schizophrenia is a cognitive disorder that affects how people think, feel and perceive things (Galderisi et al., 2018). It is appropriate to ask the patient the question as an opening statement in a clinical interview. It is important to ask the patient light questions since schizophrenic patients are unable to think properly. This disorder affects the patient’s cognitive response hence it is critical for the nurse to ensures that the patient does not get excited during the clinical interview. The nurse is encouraged to use the simplest questions to help the patient understand clearly. Schizophrenic patients also have poor emotional management and at times experience a poor relationship with others (Galderisi et al., 2018). It is, therefore, important to proceed with caution so that the patient does not harm the interviewer.

The nurse should use simple concrete and direct massage to enable the patient to comprehend the clinical interviews. It there important to ensure the patient can understand the line of questioning. Schizophrenic patients sometimes become aggressive and unable to behave because of their condition (Galderisi et al., 2018). In this perspective, it is important to use simple conversations in the clinical interview to help the patient understand the line of questioning.

The patient with schizophrenia has difficulty in relating and understanding the symptoms of the disease (Galderisi et al., 2018). The patient cannot process what is happening around them and they sometimes live in an unrealistic world around them. This type of disorder is known as paranoid schizophrenia. This patient can believe that the other people are trying to harm them or think that they are being constantly monitored (Galderisi et al., 2018). Asking precise questions, therefore, enables the nurse to build trust with the patient. In this manner, the nurse can understand the characteristic behavior of the patient's disorder.

Case 2

In the case of John's experience, the nurse has violated confidentiality. Confidentiality forms the basis of nurse-client relationship. Disclosing the patients’ information is forbidden by the law and it compromises trust and respect between nurses and patient in the healthcare settings (Penda, 2017). According to the principle of the confidentiality, the nurse can only disclose their patents information if they share data between professionals (Stockmann, 2018). The nurse can moreover disclose their patient’s information if they have obtained a written consent to permit them to share their patients information to another person. Patients confidentiality is a law that governs the doctors and practitioners to restrain from disclosing the information reveled to them during consultation or treatment (Penda, 2017). In John's case, the nurse breaches nurse-patient confidentiality when she informs her mother what had happened despite John directives.

Building trust is beneficial is to the development of nurse-client relationship (Stockmann, 2018). The nurse can promote trust through confidentiality and respect. Attentiveness is also crucial when building trust. For instance, in John's case, the nurse answers her cell phone while attending to John. At this point, the nurse had already compromised her client confidentiality since the caller could have probably heard their conversation. Furthermore, the nurse lost her attention and this compromised her client’s confidentiality since she did not follow his instructions. Arguably, the lack of attentiveness by John's nurse is probably the reason why the breach of confidentiality occurred. It is therefore important that nurses should be competent to avoid such kind of case since it can lead to lawsuits or poor performance.
