Chronic Venous Leg Ulcers

The assessment criteria for Amal’s chronic venous leg ulcers include blood and wound swab and culture to identify potential other infective bacteria apart from methicillin-resistant Staphylococcus aureus (MRSA). In addition, it is important to confirm whether MRSA causes the recent observed wound changes include pus production and a bad smell. The other details that ought to be assessed include the patient’s current weight since he has been losing weight. In addition, the body’s hydration and nutritional status should be assessed since he reports loss of appetite and this may affect wound healing in a negative manner (Collins & Seraj, 2010). The history of standing for long periods, mobility, and work history is also important in this assessment. Indicated tests include microbiological tests, epicutaneous patch test, radiologic imaging, and skin biopsy (Zipes et al., 2018).

It is also important to identify whether the observed wound inflammation is caused by an infection or it is just a case of vasculitis. Increased white blood cell count may indicate an infection.  The patient should also be assessed on whether he is deficient of and having defective heparin cofactor 2, XII Leiden, protein S, protein C, antithrombin III, and/or factor V since it may indicate untreated vascular thrombosis (Kulisic & Lipozencic, 2013). Comorbidities such as cardiovascular disease, intermittent claudication, obesity, phlebitis, and stroke should also be assessed (Vasudevan, 2014).

            Angiography and magnetic resonance imaging studies are needed to examine how vessels down to the ankle leg and to identify the extent of the obstruction (Hedayati et al., 2015). Toe digital pressure and ankle-brachial indices with pulse pressure recordings are indicated to examine the extent of perfusion of the foot (Herrington, 2014). Another indicated diagnosis is Xenon-133 clearance test to measure blood flow and to predict the chance of healing of the leg ulcers.  A blood flow rate of 2.6mL/100g may indicate likelihood of healing (Buja & Butany, 2016). Apart from receiving proper diagnosis, Amal’s psychological development and psychological needs should be addressed. Amal is at the eighth-stage of Erikson’s theory of psychosocial development (Erikson’s eight stages of psychological development, 2016). He may be disturbed by conflicting ego integrity and despair issues. Amal should be convinced that he is leaving a successful life; a move that may improve his mental wellbeing and appetite (Erikson, 1982).
