Introduction to Patient Record as a chapter has got important information and serves as the business record for a patient encounter, holds credentials and documentation of all healthcare services given to a patient, and is a depository of significant information which comprises of demographic data. While this chapter has got several topics that are imperative, Computers in Health Records is one of the most interesting subjects.
With technological revolution and several cases of human error, the use of computers outdoes the traditional paper work has been on the use for a long period of time. By definition, the Electronic Health Record (EHR) is an electronic record of patient’s health information that is generated by encounters in healthcare institutions (Green & Bowie, 2004). It records information such as patient demographics, problems, progress notes, medications, past medical history, vital signs, immunizations, radiology reports, and laboratory data. This form of data storage has got technical features which include enabling information to be shared and used over secure networks., trigger reminders and warnings, send, receive orders, results, and reports, and create more accurate billing system at the same time reduces billing process (Green & Bowie, 2004).
Electronic Health Record (EHR) has got several advantages over the traditional paper work. First, it is accurate and reduces human errors that threatened the quality of health. It reduces time (Green & Bowie, 2004). It is private and confidential; however, every country has got its own law on the duration a person’s details can remain in the system. However, it does not lack some disadvantages too. The device is expensive and difficult to use. This has enabled most health institutions to train their employees on how to use it, an activity which is not only time wasting but also expensive.
