     Ms. C, 72 years old, has diabetes mellitus, hypertension, rheumatoid arthritis (RA), and coronary artery disease. She lives alone. She recently was discharged from the hospital after treatment for an acute myocardial infarction. She had been doing well before this admission. You are the home health nurse assigned to visit Ms. C. Ms. C’s son tells you he is concerned that his mother will not follow up with all the medications she is supposed to take. He states that she has not been very compliant with treatment in the past.

        1. Discussion Board: Medication Non-Adherence

          Medication adherence is important in management of chronic diseases. Medication non-adherence may result in increased cost of care and poor health outcomes (Lam & Fresco, 2015). Method for approaching Ms. C.’s involve utilization of behavioral interventions such as cognitive behavioral therapy as well as other therapies which can treat dysfunctional cognitions, emotions, and behaviors can help modify the patient’s behavior towards treatment and improve medication adherence (Costa et al., 2015). Some of the risk factors that may interfere with Ms. C. plan of care include old age (72 years) and the fact that she lives alone. Socioeconomic variables and the cost of treatment may also interfere with the treatment plan. Besides, if care is provided by multiple providers it may be difficult for the patient to adhere with the treatment plan. Having multiple chronic disease and severity of each condition is another significant challenge (Costa et al., 2015). Important determinants of plan of care that must be evaluated include Ms. C.’s perception of the required therapy, health literacy, cognitive functioning, social support, and motivation for self-care. It should also be established whether the patient is taking multiple medications and on whether she is experiencing any adverse drug reactions, since these may contribute to medication non-adherence (Costa et al., 2015).  Medication reconciliation may also be considered so as to determine if there are potential drug-drug interactions that may negatively affect treatment outcomes or result in adverse events.
