Write a double-spaced paper with a minimum of three references (two of which are scholarly) in APA format. Include references if used, but they are not required for this paper. You will be graded on each of the above 10 items as complete or incomplete.Information and Consent for Group Participation
Entering a therapeutic group can be a highly rewarding and powerful experience. There are some obvious limitations to group work that will be outlined. Group work can also enhance individual therapy work but it is not intended to replace individual treatment for various mental health symptoms or disorders.
The expectation is that group participants commit to 8 sessions occurring every Wednesday evening at 6:00pm. You have the right to discontinue the group at any time for any reason. However, it is requested that you discuss with the group leader any issues or concerns you may have regarding the group, facilitator, group process or its members. It is requested that you notify the or if you have a prior commitment or are unable to attend group. You are allowed to miss one groups without charge in the 2-month period. After one missed group, participants are responsible for paying the full fee for the missed group.
Confidentiality including names and identifying information of all group members is to be respected throughout the course of the group and afterwards as well as it relates to information shared during group. It is also requested that group members refrain from any intimate outside relationships with one another during the course of the group series.
It is your responsibility to pay designated fess for group services at the time of group or as mutually agreed upon. This fee is agreed to be $50 per 90-minute group session. Session fees can be made with cash, credit/debit card, PayPal or check. No insurance will be filed on groups.
There are many benefits to therapeutic groups including learning new tools and skills, increased self- awareness, better relationships with others, and improved quality of life. It may be that there are periods of emotional release and/or establishing new habits and patterns that require extra self-care and/or altering your current lifestyle in order to best manage the presenting issues and possible accompanying symptoms. It is also important that you communicate with your individual counselor if additional work is needed on an issue that arose during group. It is considered the group participant’s responsibility to seek additional individual sessions in the event that it is needed.
Regarding your right to client confidentiality, you need to be aware that there are a few exceptions to your right to complete confidentiality. These circumstances include the event that your safety or the safety of another person is in question. Counselors are also required to break confidentiality in cases of abuse, court-ordered testimony, or subpoena of client records.
I have read and understood this consent form and agree to voluntarily participate in group therapy facilitated by Valerie Ismail, APRN. I have received a copy of this document if requested.

Therapeutic Group
- Type of group: Describe the practical elements of your group (closed or open, homogeneous or heterogeneous, size, meeting place, frequency, and length, short term or long term, and so on). Explain why your selections are appropriate to your specific group.
Closed group- a skilled developmental group for recovering addicts and alcoholics (adults). Since different drug addicts require specific interventions in trying to recover as well as normalize their health conditions. Heterogeneous group selection criteria were used to help reflect the society at large since alcohol and drug addiction cuts across the society regardless of the people's levels of educations, age, gender, and status among other factors. Notably, the group size selected for the research was between 25 to 30 persons. Majorly prompted by the fact that the study wanted to obtain concrete and well-detailed finding, and to help reduce the cash burden on individual persons in the case of a little size. Group meetings are to be held within the school social hall every Wednesday evening as from 6:30 pm after the normal classes’ sessions. The meetings with comprises of eight sessions running for at least one and a half hours to enable members to actively participate and give out their views on the therapy session. The group selection in this case is appropriate since it reflects atypical societal settings and heterogeneous to allow gather varied information as hold to by different societal classes.
- Topic or theme of the group-Alcohol and drug addiction recovery.
- Target population and age of participants- The targeted populations are adults age 18 years old and above.
- How and why this group is relevant to your identified population-
This group provides a supportive addition to those transitioning into recovery. Furthermore, this group is an atypical representation of a larger number of people suffering from alcohol and drug addiction within society. They would easily connote the true picture as well as a clear date that is quite relevant in developing recovery therapy steps.
- Dates and times of group meetings for eight sessions. Every Wednesday evening at 6:00 pm for the next 8 weeks
- Qualifications for the group leader, including educational level, licensure, certification, and so on. The group leader must be board certified and licensed PMHNP, LCMH, LCSW, Ph.D. Must be a master’s level prepared and above focused on counseling. The qualifications were considered due to legal and ethical requirements.
- How and where the group will be advertised/announced. Indicate how you will identify, attract, and get members for your group. How is your means of announcing the group key to identifying and recruiting group members? –
The therapeutic group would be advertised through social media, print media, and notice boards. Social media preferred includes Facebook, tweeter, histogram, and youtube since they would allow the group nature, reach as well as engage with the targeted audience who are majorly youthful population.in addition, the social media platforms enable the group to reach the targeted population regardless of their areas of location within the world (Voorveld et al 2018).
Among the social Medias platform, Facebook attracts a large population that would help the group to create brand recognition with the targeted population. This would work to the interest of the group since most people would love to associate themselves with the groups or brands that they recognize. And by using Facebook and the fact that the group targeted population are majorly Facebook users (Kreiss & McGregor, 2019). Furthermore, the use of social media would help initiate a general conversation around the therapeutic group. And in the end, this would draw the attention of the targeted population for the study by engaging them by responding to their comments and inquiries on the group. Finally, the use of social media is cost-effective. In most cases creating a social media profile and post organic information are less expensive but reach a large targeted population making it relevant for the group use (Kreiss & McGregor, 2019).
Print Medias such as magazines, flyers, newspapers, newsletters, and journals would be preferred by the group in advertising recruitment since they offer leverage credulity compared to other sources such as social media. The printed information cannot be easily altered and they are of high-quality, reliable, and credible. Most people trust printed information such that they easily get convince and this would work to the interest of the group.
The use of notice boards by the group would be effective since they are audible and can be easily read. People find it quite easy to interact with notice board information since they are direct and précised to the point hence avoiding time wastage. Similarly, they are designed in such a way that they create interest in important information that would benefit their intention to the group.
- How participants will be selected and interviewed. Identify criteria to screen participants, including drafting a screening sheet to be used by the group leader. Why is this type of screening appropriate for your group? (Include the final screening sheet as an appendix in the final group manual.) CAGE-AID see below
The group would run affair participants selected for the interview. The selection would be done in two faces, first for those who would volunteer and those who would be physically identified for the interview. The physical identification of drug addicts would be done by the assistance of the known rehabilitation centers as well as the authorities within the area of study (Levis 2019). This would help come up with a well competent team for the study that fits the targeted population age group as well as the specific addict that is alcohol and drug persons. A semi-structured interview system would be used. It would be conducted by the interviewer considering the status of the interviewees. Some of the interviewees are addicts such that they can best participate by just engaging them in a conversation.
The group would employ the following criterion in identifying participates for the interview: Perceived addiction rate, frequency of drug abuse, drug dependency, the conduct of the drug addict upon abuse, family status, and employment history. The participants would be first required to sign an informed consent procedure document before taking the interview. Below is a proposed screening sheet for the intervention (Levis 2019).

- Informed consent procedures for your group members. Draft an informed consent form. If your group is for children, include forms for both children (age-appropriate) and their parents/guardians. (Include the final informed consent form as an appendix in the final group manual.) See attached
INFORMATION AND CONSENT FOR GROUP PARTICIPANTS
Health impacts of Alcohol and drug abuse include liver serosae, termination of pregnancy, short and long term mental and cognitive disorders. Family breakage and social fights are some of the social effects of alcohol and drug abuse. Therapeutic interventions are quite necessary for the problem. Joining the therapeutic group is rewarding and profitable for the patients. Moreover, there are several limitations to the group work but on the other hand group work can enhance individual therapy work. It’s worth noting that therapeutic work does not intend to overlook individual treatment to varied mental health disorders.
A total of 8 therapeutic sessions are necessary for the group participants, with the meetings held every Wednesday evening from 6: 00 pm. An individual has the right to discontinue the group but it is ethical that one discusses with the group leader the concerns before deciding to discontinue the group. Notably, one is expected to give prior notification to commitments that my force them not to attend the group. And one would not be held liable for missing a session in two months rather they are expected to pay a fee for the missed sessions.
Therapeutic group work is based on confidentiality. Individual information as well as the discussions during the group work is not to be shared. Group members are also advised to refrain from any kind of intimate relationship amongst themselves throughout the course of the group sessions.
Members are expected to pay fees of $50 per session that runs for 90 minutes. Mode of payments is either by cash, check credit/debit cards, or PayPal since no insurance is filled on groups. Furthermore, it is an individual responsibility to pay the designated fee either during the sessions as mutually agreed upon to help facilitate group services.
Therapeutic group benefits include increase self-awareness, learning of new tools and skills, improved relationships with others, and also enhanced life quality. A therapeutic session provides an emotional release, acquisition of new habits and patterns, which demands self-care and lifestyle change to cope with the emerging issues. Participants are encouraged to communicate with their counselor on any additional work arising during the group session. It is an individual responsibility to seek additional therapeutic sessions in case the need arises.
One needs to be aware of the few exceptions instances to which individual right confidentiality can be haltered. If the safety of the counselor and other parties is questionable, the counselor is expected to breach confidentiality. This instance includes court-ordered testimony, abuse, or subpoena of client records.
I do agree to voluntarily participate in group therapy facilitated by Valerie Ismail, APRN following the reading and clearly understanding the consent form. I would willingly receive a copy of this document if requested to.
