Improving Work-flow Process for Discharge Processing
The newly created workflow is aimed at enhancing the working of coders, in a manner that will enable them to complete the coding process within three days after discharge. In order to achieve this, the old paper based process has to be compared and contrasted with the new EHR process after which redundant activities have to be eliminated in order to streamline the process and ensure that it is efficient. Destino et al. (2017) recommend the use of closed-loop process improvement. This recommendation informs my decisions and reasoning for the new workflow. The closed loop process begins with the identification of efficiency goals. In this case, the goal is to enhance the ability for coders to complete the process in 3 days. This is followed by measuring differences in efficiency goals. This entails comparing old process with the new process.
The fourth step is changing the process and coming up with an updated EHR process that achieved the required efficiency goals. The process has to be assessed and reviewed through efficiency monitoring. This involves measuring efficiency and ensuring that the process is constantly improved in accordance with required efficiency goals. In this case, the new EHR that is integrated into the HIM system is shown in figure 1 below. The processes are illustrated using a flowchart tool that divides the patient check in and discharge process into two. The first part entails activities to be performed by the patient while the second part contains those activities that will be performed by a front office receptionist. The receptionist organizes for coding, discharge analysis and filling in of charts among other activities involved in patient discharge.
The successful improvement of EHR system will help decrease DNFB. Basically, this is achieved through increased efficiency where the coders are able to complete the process within 3 days. As a result, DNFB will reduce from the current $2.4 million to a figure of $750,000. In addition, the system will reduce delays arising from physicians who fail to dictate their discharge summaries on time (Pedron et al., 2017). The real time system will enhance efficiency by ensuring that the front office receptionists oversees and coordinates these activities.
