Neuron Anatomy

A neuron consists of the cell body (soma), and a branch or branches of called a nuerite or neurites. Neurites that relay electrical signals away from the cell body are called axons while those that relay towards the cell body are called dendrites. The cell body contains organelles such as mitochondria, nucleus, and lysosomes. There are numerous rough endoplasmic reticulum in the cell for synthesis of the cell nitrite cell membrane. Most axons as well as long dendrites are covered in myelin, which have insulator function and increases the speed of electrical conduction. Myelin is made by a glial cell that wraps itself tightly over the axon leaving gaps between one glial cell and another known as nodes of Ranvier. In the central nervous system, myelin is derived from glial cells known as oligodendrocytes while in the peripheral nervous system the source of myelin sheath (Ashley & Liu, 2021).

The primary function of the nerves is to transfer electrochemical signals. Nerves classify three functional categories: motor neurons, sensory neurons, and interneurons. Sensory neurons receive and interpret sensory stimuli and relay it to the spinal cord or the brain. The intermediate neurons transmit signals between other neurons. Motor neurons relay signals to glands or muscles. Acetylcholine transmits impulses from the motor nerves to the muscle cell post synaptic membrane. The junction is known as the neuromuscular junction (Ashley & Liu, 2021). Nerve impulses are conveyed and transmitted via action potentials. Neurons have resting potential of about -60mV. Depolarization of the neuronal membrane results in an electrochemical signal. The Na-K pump repolarizes the neuronal membrane (Ashley & Liu, 2021).

How Glial Cells Function in the Central Nervous System

Glial cells known as oligodendrocytes in the central nervous system produce myelin sheath. The myelin sheath is formed when a glial cell wraps firmly around a neurite. Myelin is a membrane that acts as an electrical insulator so as to enhance the speed of conduction of impulses (Ashley & Liu, 2021). Astrocytes are other types of glial cells in the central nervous system that provide nutrients and other substances to neurons as well as regulate concentration of chemicals and ions in the extracellular fluid and provide structural support for the synapses. Astrocytes make contact with both capillaries and neurons in the central nervous system forming the blood-brain barrier (Lumen, 2021).

Synapse

An impulse is transmitted in a synapse from one axon of one neuron to the dendrite of another. The parts of the neurons that communicated with each other are the presynaptic membrane on one neuron to the post-synaptic membrane of a dendrite. The direction of impulse transmission is from a sensory neuron to an intermediate neuron and then to a motor neuron (Ashley & Liu, 2021).

Neuroplasticity

Neuroplasticity is a process that involves functional and structural changes in the brain. It is the ability of the nervous system to adapt to change in terms of its activity in response to extrinsic or extrinsic stimuli through reorganization of its functions, structure, or connections after injuries, such as traumatic brain injury or stroke. Building a Comprehensive Health History

This paper describes an interview process with a 40-year old black man who recently moved from Africa but lacks health insurance. One of the communication techniques appropriate for this scenario entails building a good relationship with the patient and establishing how fluent he speaks English since it may be her second language. Effective communication is important to any situation, particularly when physicians are conversing with patients who speak different languages. Karliner (2019) posited that cultural competency of health care providers entails understanding a person’s values and committed to offering patient centered communication that consequently can result in a trusting rapport between the patient and the health care provider. Therefore, this interview will be devoid cultural beliefs and findings. However, it will comprise of open ended questions that accurately depict various needs of the client in the case scenario. Walker, Barnett, Stauffer, & CTropMed (2020) noted that health care providers should accord patients special attention and time, particularly when dealing with patients from different countries. This helps address any challenge and allow patients to feel that healthcare providers are genuinely concerned about their health issues. After determining the language fluency of the patient, I would use simple words that can be understood by the patient. I would also keep the interview questions relatively short by using simple terminologies as possible.

Assessment of Health Related Risk

 Given that the patient is a newly immigrant to the United States, I would provide him with the necessary health resources that are in the US to enable him receive appropriate medication as he lacks health insurance.  I would also screen him for any disease that require immediate immunization. Components of health assessment for refugees and immigrants encompass handling patient health concerns, screening for diseases linked to migration history and country of origin, commencing age appropriate immunization, as well as, regular health care maintenance (MacMillan, Wathen, Jamieson, Boyle, McNutt, Worster & McMaster Violence Against Women Research Group, 2006). Moreover, due to the age, ethnicity and gender, I would perform screening for any hypertension, cardiac issue, cancer and diabetes.
