Merit-Based Incentive Payment System (MIPS)

Unplanned reoperation within the 30 day postoperative period is a problem within nursing that focuses on the inability of clinicians to properly treat patients during surgical operations. Ideally, most of the complications are can be done in one operation. However, lack of adequate patient safety by nurses leads to complications which necessitate another operation. The issue is significant to clinical practice since it results in poor outcomes for patients, mainly when time wastage, and costs occurs. Similarly, unplanned reoperation within 30 days after another operation is essential to improving clinical practice as it emerges from limited observation of patient safety.

The main reason for selecting the issue is its extreme impact. Ideally, it is an adverse surgical outcome, which is often a preventable cause of harm, thus it is important to measure and report.

 The primary objective of implementing a merit-based incentive payment system (MIPS) is to improve the overall quality of care offered to patients by clinicians. As such, positive scores are offered for carrying out practices that promote positive patient experiences based on improved quality of care. Principally, eliminating incidences of unplanned reoperation within the 30 day postoperative period in healthcare calls for improved care of patients. Ideally, it promotes the use of patient-based care. As such, it helps meet or promote the objective of MIPS which is increasing the quality of clinical care. As such, it improves the nature of patient experiences in healthcare. 

Katakam and Suresh (2017) explain that quality improvement requires four significant steps which comprise addressing the high priority areas following institutional safety and quality priorities, complying with appropriate regulatory requirements, and building capacity to continuously monitor as well as improve the safety and quality healthcare services.  Since unplanned reoperation within the 30 day postoperative period is a high priority area, especially relating to adverse outcomes from surgeries carried out on inpatients, it falls under the criteria described by Katakam and Suresh (2017). Therefore, subsequent procedures of following institutional issues, complying with external requirements, and building capacity to manage the issue can apply effectively.  Therefore, the management of the issue can be done under Katakam and Suresh’s (2017) criterion.
