    Nursing assignment    

    Find a potential client (friend, family member, current training client, yourself) and initiate an initial assessment.

    Have the client complete the appropriate questionnaires. Forms can be found in the “Initial Assessment and Triage Questionnaire” packet included with your course materials.

    Design and fully describe your nutrition and coaching plan for the client including:

    1. Your findings from collecting preliminary client information

    2. Client level and the reason for choosing this level

    3. Limiting factors

    4. Outcome goals

    5. Behavior goals

    6. Which assessments you will record and why/how you will incorporate the results

    7. Your recommended nutrition and supplement plan

    8. What (if anything) your client will need to discuss with his or her physician

    9. Referrals to your professional network (if needed)

    10. Proposed appointment scheduling including:

    a. Frequency

    b. What you will discuss at each appointment

    11. A plan of action if you observe a plateau

    12. A plan of action if your client changes his or her goals

    Be as detailed as possible in your plan. Can be 300 to 550 words. Attached is also what I started. needs to be with plateau.

    I am doing this case study on me. I completed some questionnaires. I did the assessment & triage and athletic Nutrition questionnaire on me. Going to doc today to do a baseline blood chemistry. I will probably fill more forms as I go.

     

    In the goal section my goals were to increase muscle mass and loose few more pounds. Kind of went backwards regarding weight. My health problems are levoscoliosis centered at L3. An anterolisthesis at L3-4 and minimal retrolisthesis at L4-5 and bulging disc at T12-L1 and more. . Otherwise bad back! It pinches so bad it hurts my left calf to walk.  My nutrition habits are normally I say a 9 on the form. I get that one day a week cheat meal. I work out 6 days a week, sometimes twice a day. At least an hour lifting weights and 2 hours cardio right now to get the weight down(10 lbs). When the shot is working. Had 4 this years and need one now.  Live alone and do cook all my meals including protein shakes.  I am going to start at Level 1 I think. Stress and anxiety high.

     

    Going to increase what I eat in the morning b4 training. At first nothing then a protein shake but going to promise myself to increase to a bowl of oatmeal or rice and chic. Something slow releasing carb. Afterward always been pretty good about eating or protein shake afterwards. Being alone always eating in front of the tv and fast eater so going to practice putting my fork down in-between bites. Throughout my work out I drink.

    Preliminary Information

    I am doing this case study on myself. I did the assessment and triage on athletic Nutrition questionnaire on myself. My health problem include levoscoliosis centered at L3, an anterolisthesis at L3-4, minimal retrolisthesis at L4-5, and bulging disc at T12-L1. Regarding bad back, it pinches so bad and it hurts my left calf to walk.  My nutrition habits are normal; I say a nine on the form. I am fine controlling my appetite, which allows me only to indulge when necessary. Moreover, I am aware of the foods I eat as I am cognizant of food portioning and levels that are ideal for fitness. My goals are to increase muscle mass and lose a few more pounds. My levoscoliosis, anterolisthesis, minimal retrolisthesis, and bulging disc affect my workout routine as they cause a bad back that hurts my left calf when walking. However, with some help regarding nutrition and fitness, all of my goals can be achieved.

    Client Level

    I would classify myself as being at level 3 category mainly due to my awareness of proper nutrition intake regarding risks and benefits. Moreover, despite my health concerns,   I am committed to routine workouts.  Level three is challenging to maintain as one tries to make adjustments while getting better. As such, I feel that it would be ideal to find a coach who will help me optimize my fitness and nutritional engagements instead of doing it alone as I can harm myself while trying to maximize outcomes. 

    Limiting factors

    The identified health concern consisting of levoscoliosis centered at L3, an anterolisthesis at L3-4, minimal retrolisthesis at L4-5, bulging disc at T12-L1, and bad back are the limiting factors. Therefore, a routine check-up with my physician is recommended, especially if anything out of the ordinary happens.

    Behavioral and outcome goals.

    The outcome goals comprise losing a few pounds while also ensuring that my micronutrient level goes up.  The approach will be realized by having behavioral goals such as increased consumption of vegetables and fruits while maintaining a proper sleep of about 7 to 8 hours each day

    Nutritional plan

    The recommended nutritional plan will look like;

    Breakfast

    1. Oatmeal: one serving
    2. Eggs: one serving
    3. One banana

    First Snack

    1. Water: 8 oz.

    Lunch

    1. Portioned meat salad
    2. Water: 8z
    3. Apple: one
    4. Protein Shake

    Second Snack

    1. Water: 8 oz.
    2. Rice cake

    Supper

    1. Rice: One serving
    2. Broccoli: one serving
    3. Chicken: one serving
    4. Salad: one serving
    5. Water: 8 oz.

     

    Assessments to Record

    I will record both the nutritional and fitness assessments as a means of creating a future reference. However, I will focus on the initial health recordings and my food logs. The initial records will be useful in comparing any health changes, while the food logs will help develop a statement regarding the impact of food intake on health results.

    What to discuss with physician and appointment scheduling

    I need to make bi-weekly appointments with my physician to discuss how I manage my health plans and my adherence to the nutrition plan. Principally, the bad back issue should come up often as I look for solutions from the physician. Moreover, I would seek to improve my accountability levels by discussing the value of a fitness and nutrition routine with the physician.

     

    A plan of action for a Plateau

    Regarding a plateau, I recommend a change of diet due to specific fitness needs. However, I feel that a plateau would not be ideal at the moment as I prefer observing a change in routine to help my body avoid any halts associated with a plateau as it remains confused (Buckworth, 2017).

    Plan for Switching Goals

    If I decide to switch goals, it will be ideal to develop another nutritional plan to help implement a new routine. The training routine will also change from six times a week to a suitable frequency based on the newly identified goals.
