Patient’s Health History Taking
History-taking is fundamental to effective patient care. While taking a health history, the doctor should introduce themselves and confirm they speak to the correct patient. Building a health history involves the physician simultaneously attending two narratives –one from a biomedical perspective and another from a patient's perspective. The two work to provide a complete and contextualized history. History building uses questions asking mindfulness where the physician starts with open-ended questions and gradually shifts to close-ended questions until they have specific information they need. Organizational multitasking in history building requires the physician to listen to the patient's narrative organized around the patient's life and manage the biomedical aspect within the diagnostic framework. The use of conversational devices other than questions such as directives during information sharing helps build health history.
Asking difficult questions is facilitated by the use of empathic statements to strengthen the clinical encounter. The use of empathic statements facilitates patient-physician communication, trust, and disclosure of information. Using non-judgmental ideas also makes the patient more often open up to complex questions. Prefacing questions that might trigger a solid adverse reaction can reduce upset; by asking difficult questions, the trainee should anticipate needing to explain the question’s purpose.
