Mr. Y is receiving penicillin for the first time. He was diagnosed with gonorrhea. He is to receive the parenteral form of the drug penicillin G. You are the nurse responsible for his care.Mr. Y states that he has never received penicillin before. Is he at risk of developing a reaction with this first dose?
Mr. Y asks why he needs an injection. He would like the PO form because he does not want to have an injection. How should you respond?1.As Mr. Y's nurse, I would introduce myself and explain my job role. I would go over his laboratory
results and explain his treatment plan. Mr. Y has been diagnosed with gonorrhea and has been
prescribed penicillin G. Mr Y seems a little concerned over getting penicillin for the first time. I would
recommend a penicillin skin test (PST) that could be done with a allergy specialist.
Following PST, IgE -mediated reactions typically occur within 1 hour of exposure but may occur
up to 72 hours after-ward; such reactions may include angioedema , urticaria, shortness of
breath, rash with pruritus , bronchospasm , and anaphylaxis ( Alharbi , H. A.(2019). Mr. Y would be
able to be monitored more closley and can be rapidly helped if an emergency were to happen.
It is very important for Mr. Y to receive the medication. Most infections are asymptomatic,
and untreated infection may lead to severe reproductive sequelae , including pelvic inflammatory
disease and infertility among women and epididymitis among men (Learner, E.R., at el., 2020).
Penicillin g is not recommended to be taken by mouth because it does not absorb properly.
Theinjection will help speed up the process and help Mr. Y get ride of the infection.
- As Mr. Y’s nurse. I would inform him what the antibiotic Penicillin is and what it does to the body which is, a
β-lactam antibiotics that disrupt cell wall formation and integrity by targeting the major penicillin binding proteins
(PBPs), mainly PBP1 and PBP2 in gonococci (Kivata et al., 2020). With that being said, there are common side
effects when taking penicillin such as nausea and diarrhea. I would let Mr. Y know that since it is his first time
receiving the dose, there may be a reaction, since he has not been exposed to it before. He must contact the doctor if
having any symptoms.
I would explain to Mr. Y. that, while I understand that he would like the PO form, the intramuscular administration
injection of Benzathine penicillin G is slowly released, combined with hydrolysis by penicillinase-producing
bacteria, it results in the maintenance of blood levels that are much lower than those of other parenteral penicillin
and a prolonged duration of action (Hassan et al., 2019). As his nurse, I would ask Mr. Y. if he could stay in the
office after administration for observation to check for any reactions, to make sure he feeling well to leave and to
ask any questions if any.

I agree with the first post. By the nurse introducing him/herself and explaining his/her role, this step enables Mr. Y to develop trust towards the nurse and understand the procedures that will be suggested by the healthcare provider. Furthermore, since Penicillin might have severe allergic reactions, it is important that the nurse considers recommending a penicillin skin test given that it is the first time Mr. Y is using this medicine (Hassan Alzahrani, Abdelmageed Imam, Mohamed Ibrahim & Palaian, 2019). The first post is valid since it provides essential information that Mr. Y needs to know. For instance, the nurse takes the step of informing Mr. Y of different signs and symptoms that might emerge after taking the medicine. Moreover, the nurse explains in detail the possible duration that it might take for Mr. Y to start experiencing the medicine’s reactions.

I also agree with the second post given that the nurse takes the obligation to explain to Mr. Y about Penicillin. This step is vital since most patients lack information about the medications they take (Hassan Alzahrani et al., 2019). The nurse not only informs the patient about the medicine but also explains to him why the medicine must be administered through injection. The second response is valid given that the nurse suggests that if it is possible for Mr. Y to remain in the office after administration of the medicine for observation to identify if there is any reaction (Conway, 2017). This step essential since it allows the nurse to ensure if Mr. Y is feeling alright and ready to leave. Moreover, the second post is valid since the nurse allows Mr. Y to ask any questions regarding the prescription.
