Although the current U.S. health care delivery system offers undeniable benefits, according to many, the negatives are becoming increasingly unacceptable to greater numbers of people. Rising costs, consumer activism, and a changing model of health care delivery are driving health care reform. The movement to reform health care has stimulated great political and social interest, promoting debate and action at local, state, and federal levels. Those in the nursing profession can play a pivotal role in the reform movement.
This week, you will consider the role of nurses as leaders in health care reform as you examine the Institute of Medicine’s recommendations for areas in which nursing education and training need to be strengthened to better prepare nurses to be effective leaders.

As health care delivery in the United States continues to evolve, either through mandates, improved technologies and training, or other drivers, nurses remain at the forefront in facilitating the success of new initiatives. In 2010, the Institute of Medicine formed a committee of experts to address the following question: “What roles can nursing assume to address the increasing demand for safe, high-quality, and effective health care services?” In the final report of the committee, The Future of Nursing: Leading Change, Advancing Health, the authors stated:
This report offers recommendations that collectively serve as a blueprint to (1) ensure that nurses can practice to the full extent of their education and training, (2) improve nursing education, (3) provide opportunities for nurses to assume leadership positions and to serve as full partners in health care redesign and improvement efforts, and (4) improve data collection for workforce planning and policy making. (p. 10)
For this Discussion, you will focus on the research recommendations presented by the committee concerning the role of nurses as leaders. The committee members believe that answers to these research questions are needed to advance the profession of nursing and to further expand their role in health care reform and improvement.

Review Chapter 7 in The Future of Nursing: Leading Change, Advancing Health report provided in the Learning Resources. Focus on the information in Box 7.3, “Research Priorities for Transforming Nursing Leadership.”
Select one of the research priorities listed in Box 7.3 that is of particular interest to you and applicable to your career interests. Consider the benefits and challenges of researching and addressing this priority in nursing.
Using the Walden library, identify two to three current articles that address your selected research priority. Consider the current state of research efforts on this priority.

Week 4: Population Health – Part 1
Population health encompasses health outcomes of a group of people or individuals, including the distribution of those outcomes/consequences within the group. These outcomes include mean mortality in terms of ethnicity/race, socioeconomic status, geography, and gender. Health-associated quality of life is another outcome which can also be measured in terms of socioeconomic status, ethnicity/race, gender, and geography. Determinants of population health include healthcare, social environment, genetics, physical environment and individual behavior (Kindig, Asada, & Booske, 2008). This paper presents shortage of nurses as a population health issue since it determines access to health care and services.

Shortage of nurses is a health care determinant that can affect access to health care services and even result in poor healthcare outcomes. There are about 29 million nurses globally of which 3.9 million are working in the US. About 1 million additional nurses will be required globally by the year 2020. In the US, several issues contributes to nurse shortages. Aging nurse population is one of the challenges. About 1 million registered nurses are aged 50 years and more in the US. By 2029, all nurses of the baby boomer generation are expected have retired from the profession. The second challenge is that the turnover rate for nurses in the US is 8.8 to 37% depending on nursing specialty and geographic location (Haddad & Toney-Butler, 2018). It has been established that positive relationships exists between nurse shortages and workload. Besides, negative correlation exists between workload and quality of patient care (Shammika & Alwis, 2015). Therefore, nurse shortage is a population health issues that must be addressed.
