Development of a computerized shuttle transport request/scheduling system
Work with current EMR provider (EPIC) to develop this scheduling system with as many similarities to current scheduling system used within the organization as possible
Development of the scheduling system within the EMR will allow for patient tracking and easy documentation regarding discharge, transport and receiving of patient from department to department (UC to ED)
Institutional Structures
Continuous Improvement Team at Elliot Hospital, whose role is to lead and facilitate improvement projects within the organization (Elliot Health System, 2018).
Information technology (IT) department at Elliot Hospital will need to work directly with the EMR developers to create a program
Policy development team at Elliot Hospital will need to update the current transportation policy to include the use of this new system
Once the program and policy have been developed, IT and the education department will need to work together to create a training program for staff regarding the shuttle scheduling system
Financial Trends
Elliot Health System recently completed the construction of a new medical office building which opened in August 2018
Additionally, Elliot is in the early stages of several construction projects/updates to the main hospital campus, including a new parking structure and addition to the main building to expand services for and inpatient rooms for patients
These large scale projects are taking on much of the financial focus of the organization currently which could be problematic to my proposal
However, I believe up front capital for my proposal will be minimal as Elliot already owns several shuttle buses in use at the main campus to transport staff and patients from offsite parking to the main campus or other services.
I propose utilizing an already owned shuttle during the initiation of the UC to ED shuttle service, therefore no additional cost will be incurred for this aspect of the proposal
While some investment will likely be required in the addition of a shuttle scheduling service, as we already contracted with EPIC this will also be minimal and may be able to added to one of the routine updates occurring several times annually
The biggest cost I anticipate is the hiring of shuttle driver(s) to operate this service
I would propose the utilization of properly licensed volunteers to operate the shuttle as a cost savings measure
Policy Restraints
I see no restraints to my proposal related to governmental or hospital policy
Regulatory Requirements
Any driver of the shuttle bus, volunteer or employee, must obtain a NH CDL driver’s license to operate a passenger shuttle bus (DMV, 2018).
The driver will be subject to organization requirements including employment physical with vision screening, drug screening and maintaining a clean driving record.
The shuttle bus will need to complete annual inspection per NH state law and maintenance records will be maintained
Patient diversity
Shuttle service will be offered to any appropriate, non-emergent transfer patients at the UC regardless of race, religion, gender, or sexual identity
Cultural sensitivity training will be provider to all drivers and staff involved in patient care (Health Research & Educational Trust, 2017)
Information will provided in different languages as necessary, including the use of the currently established telephone interpreter line utilized within the organization for clear explanation to patient regarding the shuttle service when being utilized (Health Research & Educational Trust, 2017).
Building Buy-In

3.6 million people in the United States do not seek medical care due to transportation barriers, in providing this transportation service Elliot can better serve our community, increase wellness and healing (Health Research & Educational Trust, 2017).
Increased availability of EMS/ambulance for true emergencies will provide not only a cost savings, but also increase patient safety within the community
Increased efficiency in transporting non-emergent patients will free up beds in the UC and decrease wait times for patients and increase nursing staff satisfaction and decreasing risk associated with care provided to patients who present to the incorrect setting (UC rather then ED)
The development of a transfer center to coordinate scheduling of patient transport resulted in a $55,000 annual savings in one study, this cost savings (Hains et al., 2011).

The implementation plan covers all the necessary aspects including technological considerations and policy related issues. Indeed, technology plays a central role in transforming the shuttle scheduling system to include internet enabled request and scheduling approaches which will enhance efficiency in service delivery, allow for monitoring the activities in a real-time basis, and promote patient safety. Other critical aspects in the implementation plan are a collaboration between the IT department and EMR developers to create a program that will suffice all the organizational needs. To contribute to the discussion, although the post contends minimal costs in implementing the proposal, it is important to consider that activities such as hiring and training employees require financial resources. In implementing educational programs, some of the costs incurred are providing incentives to involved individuals and obtaining the necessary training materials. Besides, collaborating with the EMR developers requires finances as this is a business to them. Another essential aspect to include in the implementation plan is how to reduce resistance to change among the employees. While some may support the shuttle scheduling system improvement, there are those that may reject it based on the fear that it may affect their jobs. Therefore, this may be factored in as a constraint in the implementation process. A strategy to address the problem is adopting a profound change theory such as Kurt Lewin where in the unfreezing phase, the stakeholders are enlightened about the importance of the change (Batras, Duff, & Smith, 2016). This phase as well as involving them in developing the change and its implementation draw their support which is critical in the success of the new initiative by the organization. Regarding the policy and regulation requirements, all healthcare transportation systems must conform to the transport laws and regulations which should be factored in the implementation plan. This ensures effective shuttle conditions which further promote patient safety and efficiency in service delivery.
